At Victoria Medical Centre we currently hold a list size over 11,500 patients. 

You can register as a patient with the practice by completing a registration form which can be obtained from reception or by clicking the PDF form below or via the www.nhs.uk/nhs-services/gps. 

If you are to fill a registration form out in reception staff may ask you to have a seat and wait if you attend during a busy period before they can dedicate the time needed to check your registration form. If you require assistance with filling the registration form out please make the patient co-ordinator aware and they will assist you with this. 

It usually takes around 5 working days for your registration to be processed, to avoid any delays please ensure you complete the entire form to the best of your knowledge. Forms may be rejected if important information is missing.  It is your responsibility to source the information required. We are a safe surgery for everyone in our practice area. No ID or proof of address is required for registering, we may ask for these if we have reason to think you have moved out of our practice area. 

You will be asked to complete a questionnaire which will provide us with important information regarding your health.  

All patients that take repeat medication will be required to have a telephone consultation with the GP. Please ensure you have a months supply of medication from your current GP before registering with us.
